intermittant rough running

my buddy has a 91 5.0 and when you first start it runs really really rough and sounds like it has a fat cam in it, and you usually have to give it gas to keep it running. but after like 15 seconds it runs fine. anyone have this happen before? were gonna take a look at it later on, but just looking for some advice
