P0191 help please

P0191 help please
My car was jerking when I would give it gas when it was hot but when I would let off the gas it wouldnt do it only when I would give it gas and then it threw a code p0191 any help please and thanks. Oh I changed my fuel filter about 20k ago I have 80k right now

P0191 Fuel Rail Pressure Sensor Circuit Range/Performance
Probably either the pump or filter is clogged or something. Start with the filter and then check into the fuel pump. Either that or anyone else have ideas what could cause that kind of error to throw?
Also do you have a custom tune on it? If you do I would take it off for now and I would also data log with the tuner.
If you dont feel comfortable doing this you can take it to ford. They charge 99 dollars to diagnose a problem out of warranty.
