Case of mine, actually pumping enough at times. As to the actual mechanical cause, someone else would have to answer as I have no definitive answer.

In the case of mine, it was simply defective. **** happens really. Never worked perfectly since I bought it, until I finally convinced the dealership it wasn't all in my mind. Been great since they finally replaced it. No loud straining, dieing at times, etc..
