Default stalls when put into gear

if my car is in park or neutral, and i put it in drive/reverse it reallly wants to stall. i can keep it alive by giving it some gas while shifting. the weird part is that this is intermitent and only happens prolly half the time. doesn't matter if its warm or cold. and when in drive, it drives with no problem and doesn't want to stall at all. any idea's?

DIY KOEO/KOER self tests... you may also have a problem with the converter, LUK what shows up from the self tests.
