your throttle body could have something stuck, or some sort of build up. check your intake system, could be the reason why youre running rich. did this happen all of a sudden?

Won't be a vac leak if it's surging. Surging is due to a rich condition, meaning, not enough air is getting in, or too much fuel. A vac leak allows more air to get in, causing the idle to go up.

Try creating a vac leak and see what happens. Just pull a vac line off with engine running and see if it cleans itself up a bit. Could be your IAC not allowing enough air into the engine.

with a gauge...does it ever clear up for a little while?

A vacuum leak while cause it to smell like fuel out the pipes at idle.

They will also idle high at times and surge at others.
