Default Please help daily driver having major issues

Hello all I’m new to this forum but don’t know where else to turn. I have a 2006 mustang GT 4.6 with just a few basic bolt on performance parts. Last week I got the p0345 code along with some very rough acceleration and almost stalling at low rpms, when I go WOT it hesitates but then takes off just like everything is normal. so I went ahead and replaced both cam position sensors and even the crankshaft position sensor just for the hell of it but to no avail, I’m lost on what to do now and I drive this car 45 mins to work and an hour with traffic home so I really need to get this figured out. Any suggestions would be very much appreciated! Thank you.
