Sure there have been engine failures. Mostly due to just "trying it out" before getting it properly tuned after an install in the garage. Others, from pushing it beyond the limits in all out racing.
All the major S/C manufacturers have kits that are safe on stock engined Mustangs.
I sell Prochargers if interested. (Just thought I would pimp myself a little bit)

it happens... if you get a good tune and stay around the 450 whp and 10-11 psi boost area you should be good for a long time. there's no assurance but that seems to be the safe area, many have run more with out issue but you never know
