I would go ask around on the Team Shelby site and see if they can direct you to the "right" person.

 

My 2010 didn't come with one and I don't don't even think it was an option. Here's the link if anyone wants to get one. The site says it only for a 2007 but that doesn't make any sense since there was no major interior mods from 05 to 09; I think the site is just outdated. They are personally signed by Carol Shelby. Since he's 85 (I think) I don't see them offering them too much longer.

I think they came on all the Shelby GT's, but not the GT 500.

 

You are right the Shelby gt comes with a plaque however it is not hand signed by Shelby . It is stamped... The hand signed plaque is ordered through SPP
although my dealer offered it as an option when I ordered my car through them ... They just basically ordered it as I could have

 

The OP has a Shelby GT, all of which came with a serialized number. GT500's are not made at Shelby so don't come with that plaque.
