Default HELP!! Car hesitates and I got 3 codes on OBD1

first off.. this is on a 1994 Mustang gt 5spd.

3.73 gears
full exhaust(headers, cats, catback)
bbk cold air intake
brand new MAF
cleaned IAC yesterday
ran injector cleaner
70mm TB

this problem has been going on since I bought the car(saturday)

I have been having a problem with this car. At around 5k rpm it just won't climb any higher in the rpm band and hesitates/chokes.......

Today I ran a test with the OBD1.. the codes were 332, 314, and 311

332= insufficient EGR flow detected
314= Thermactor air system//fault during engine run self-test/ left side
311= thermactor air system/fault during engine run self-test

I am not 100% sure I ran the test correctly. Someone told me a while back that you have to disconnect your pcv?? Are they correct?? If so, where is it on the 94 stangs?
