I'd recommend changing it at the earliest opportunity.

When the throw-out bearing makes noise the bearings are well worn. Depending on how the throw-out bearing is made, it's possible that it could lose bearings and fail.

When it does fail, you'll not be able to take it out of gear.

This is what I would suggest. Oil change, radiator flush, trans flush, fuel filter and air filter change, new plugs, check the coils. Fresh gas with injector cleaner or fuel system cleaner. And as said before visually inspect the engine.
